In a perfect world in which money was not such a powerful entity, if having 2 NESA devices was covered by the public health system, we would have an unbelievable waiting list, because I think that's one of the major problems that we have in public health, which is how we rest.

I am currently working at San Eloy Hospital as a General Surgeon, mainly laparoscopic, but I also deal with emergencies. And I do a job that I love and it's beautiful at the Hospital San Juan de Dios, also in Santurce, as a coloproctologist and surgeon. Whenever someone asks me what I do for a living, I always say that I have the most beautiful occupation in the world because I think it's wonderful. We always go against something that is inevitable, because no one stays here, but we surgeons try to make our stay as long as possible. So, it is a matter of providing quality of life to patients, especially with regard to the surgery we perform, which is colon oncology.

The pelvic floor has been a great unknown, especially because women even gave birth at home. So, the pelvic floor pathology, the pelvic floor descent with the incontinence due to the episiotomies that were carried out, due to the corresponding great parity, for women it was absolutely unknown. Instead, we have opened our minds a bit and women have become empowered. So, you receive many patients with a lot of incontinence that actually complicates their life, because incontinence is a social problem.

We have lived through a pandemic, something we just couldn't believe. It seemed a minor issue, we didn't think we were going to live through it, but people have come out with pathologies that we probably did not know about before.

I always ask patients: Do you sleep at night? Always. It is a major concern, because, well, I am going to emphasize something, if I may. I am a big fan of Fito, and Fito has a part in one of the lyrics of his songs that says: Problems seem much bigger at night. When you don't rest well, everything is much worse the next day. Resting well is fundamental.

When you make a diagnosis of colorectal cancer I always ask them: Are you resting well? And they tell me: Of course not. Because that autonomic system is absolutely stressed out. Therefore, resting well is essential.

And I tell Laura many times: Sure, these patients I receive, they already come under medication. I mean, to what extent is a treatment based on pills valid when we can resort to other such innovative treatments that are less invasive? That is fundamental, to be less and less invasive and more and more decisive. I think that's why NESA, when I was told and explained about it, seemed fundamental to me. Because there are so many insomnia problems, that is, they are somehow in a state of alert all the time. That autonomous system is all time running and does not let them rest well. It is not a restful sleep.

I consider that medicine is one thing and day-to-day life is another thing. A restful sleep, that they wake up and they don't feel tired. Of course, when I tell such big news as "I'm going to operate on you for colon cancer. We are going to try to save your life so you can go on", I always ask, "Do you sleep well at night?" Of course, because problems are not the same, they are not faced in the same way with a good night's sleep as they are without sleep.

Well, and last, I want to thank you for this opportunity to speak and to explain about pelvic floor pathology and insomnia. I think they are pathologies that, well, it's not that they have been kept hidden, they were simply not commented on or people did not dare to talk about them. I think it is important nowadays. If something happens to you and there is a solution, go to the appropriate centers, and to this specialized center where they can solve your problem, and it's important because it is a pathology that can change the way you live and above all the way you are in your social relationships.
